Department of Transportation Exams

All drivers, interstate or intrastate, are required by the federal government to pass a medical examination conducted by a licensed healthcare professional prior to operating a commercial vehicle. This must be renewed every 2 years or more frequently depending on existing medical conditions.

After May 21, 2014, all CMV drivers who are required to be medically certified must be examined and certified by a medical examiner listed on the National Registry.

Dr. James Herzog is a Certified Medical Examiner who has completed the training and successfully passed the Medical Examiner Certification Test through the FMCSA.

Our office provides convenient, cost-saving DOT Medical Exams. With same day appointments available. Our fee for the DOT exam is $125.00 dollars cash or credit, with a $25.00 dollar re-exam fee if needed. We do not participate with

insurance for DOT physicals. We will give you a receipt which you can, in turn, submit to your insurance company for possible reimbursement.

Upon your first visit, Dr. James Herzog, a Board Certified Physician as well as a National Registry Certified Medical Examiner, will conduct a thorough history and physical examination. Including but not limited to a Vision test, Physical exam, Urinalysis and a review of your past medical history.

What do I need to bring to the exam?

You must bring your driver's license, if you are coming in for a recertification you must bring your past DOT exam card also. The Medical Examination Report For Commercial Driver Fitness Determination form will be provided by our office as well as the Medical Examiner's Certificate to qualified drivers after the exam is performed.
